What's the Process For Sending Our Employees to True Phoenix®?
  • Complete all sections of the Employer Authorization for Treatment Form.
  • Have the form signed by an authorized representative.
  • Send the completed form with the patient on the day of their visit.
  • Contact a True Phoenix representative for other ways to provide authorization.
What Should My Employee Bring To The Appointment?

Your employee should bring:

• Valid Driver’s License, State/Federal/Local Id, or Employer-Issued Badge
• Authorization Form (document, text, email)
• Any medical documentation needed for a Department of Transportation (DOT) physical.

Do You Provide Mobile Services?

Yes. We will perform drug testing, DOT physical exams, Flu testing, TB screening, and other services for companies who have 10 or more employees scheduled.  *Mileage fees may apply. Contact us to discuss our other occupational health services for employers.

Do You Accept Walk-Ins?

No, we do not accept walk-ins at this time. Appointments can be made online or over the phone.

What Drugs Do You Test For?

We can currently test for:

• Cocaine
• Marijuana
• Opiates
• Methamphetamines
• Phencyclidine (PCP)
• Barbiturates
• Benzodiazepines
• Methadone
• Methaqualone
• Propoxyphene
• Hydrocodone
• Oxycodone
• Hydromorphone
• Oxymorphone

How Long Will It Take to Receive The Test Results?

For standard UDS tests, a negative result may take up to three days. A positive result may take seven to ten days because the medical review officer calls the donor to verify prescriptions. The donor has five days to call the medical review officer back.

Negative rapid urine drug screen (UDS) test results are sent to the employer within one hour of the screening. In the case of a positive rapid UDS test result, the employer will be notified the same day that it has been sent to the lab.
The lab can take anywhere from three to seven business days to send results.

Hair test results may take between one to three business days.

Alcohol tests are reported to the employer on the same day.

How Will Employers Receive the Test Results?

Employers can choose to receive their results via fax or email.

Will The Employee's Medical Records Remain Confidential?

True Phoenix® will not share an employee’s medical information without authorization. Only test results will be shared with the employer.

Does True Phoenix® Provide Hair Testing?

Yes, we offer a non-DOT test, a 5-panel with expanded opiates, and a 13 panel with Fentanyl.

Does True Phoenix® Report to the Motor Vehicle Administration (MVA)?

No, the driver is responsible for submitting all documents to the Motor Vehicle Administration.
